The Witcher 2: Assassins of Kings

This sword is only available in Dark Mode, as part of the Kinslayer's Outfit.

The Witcher 3: Wild Hunt

Weeper returns as a silver sword in the third game, in two versions:

Weeper (crafted)

Weeper (relic)

It can only be found as random loot inside caves in Velen and Skellige, and from guarded treasures.
